이병헌이 ‘아이리스’ 11회에서 방영된 성당 총격신을 촬영할 당시 총이 발사되며 나온 화약이 자신의 왼쪽 눈에 들어가 자칫 큰 부상으로 이어질 뻔한 것. 사고 당시 이병헌은 눈을 뜨지 못하고 계속해서 눈물이 흐를 정도로 심각한 상황이었지만 스태프들의 응급 처치로 다행히 큰 부상으로 이어지지는 않았다. 해당 장면은 극중 선화(김소연)에게 부모님의 암살에 대한 얘기를 들은 현준(이병헌)이 자신을 키워준 신부를 찾아가는 장면에서 백산(김영철)의 부하들과 총격을 벌이는 신이다.
제작진은 총격이나 액션 장면이 많아서 안전사고에 항상 대비하고 있었지만 이병헌의 부상 사고 이후 안전에 더욱 각별한 신경을 기울이고 있는 것으로 알려졌다.

In a gun battle scene at a Catholic church in 11th episode, Lee, Byung-Hun got gunpowder in his left eye, which could have been led to a severe injury. The accident appeared serious as Lee, Byung-Hun could not open his eyes with tears trickling down; however, thanks to the staff’s emergency treatment, the injury didn’t lead to a bad one. The above-mentioned scene is where Hyun-Joon (Lee, Byung-Hun) hears about his parents’ assassination from Sun-Hwa (Kim, So-Yeon) and gets involved in a gun battle with Baek-San’s (Kim, Young-Cheol) men on his way to the priest who brought him up.
It is said that the crew members have always been taking safety precautions as there are many gun battle and action scenes, and after Lee, Byung-Hun’s accident, they are paying extra attention to safety.
